Start with a simple timeline
Write down the date, city and type of incident. Separate what you personally remember from what someone else told you. Note any medical care, insurance contact or report you already know about. You do not need to decide who was legally responsible before asking for help.
Keep your records together—not in this request
For your own preparation, make a list of reports, photos, medical bills and insurance correspondence you already have. Keep the originals.
